1292 Molecular phenotyping of keloid skin samples suggests polar immune dysregulation

Keloids have been classically considered an aberrant wound-healing response, particularly common in Black or Asian populations. There are currently no targeted treatments for keloids due to their elusive pathogenesis. To further support immune-basis hypothesis keloids, we performed global phenotyping of keloid patients (n=27) and controls (n=23), primarily African Americans, using RNA-Seq Olink proteomics biopsies from lesional, non-lesional, control skin. In addition significant dysregulation wound healing epidermal barrier genes, found upregulation key Th1, Th2, several Th17 immune pathways including biomarkers that therapeutic targets (e.g. OX40L, OX40, IL4R, JAK3; FDR<0.05). We also correlations between clinical characteristics data, such as number correlating IL-19 IL-2 levels (r >0.5, p<0.05). Similar trends were observed the proteomic analysis, which highly correlated with showing OX40 IL-4R (FDR<0.05). These findings suggest possible link skewing, a higher may benefit systemic immune-based treatment.
